Finding the right assisted living community for a loved one is one of the most important decisions a family will make. Nexus Transitions is here to make it clearer, calmer, and more confident.
Start the ConversationMaybe your loved one is nearing the end of their stay in a skilled nursing facility, and the discharge planner has given you two weeks to find somewhere to go. Maybe your parent has been managing at home but things have changed, and you're trying to figure out what "assisted living" even means. Maybe you've toured a few places and you're more confused than when you started.
We've heard all of these stories. And we've helped families find their way through every one of them.
Nexus Transitions is a senior living placement agency โ which means we do the legwork of finding and evaluating facilities so you don't have to. We sit on your side of the table. We ask the hard questions. And we don't recommend a facility unless we genuinely believe it's right for your family.
Our placement services are paid by the facilities when a resident is successfully placed. You get a knowledgeable, experienced advocate โ at no charge.
We serve families across San Francisco Bay Area and Greater Sacramento with deep, on-the-ground knowledge of local facilities.
We sit on your side of the table โ not the facility's. Our job is to find the right fit for your loved one, full stop.
Before we suggest anything, we learn about your loved one โ their medical needs, their personality, the things that make them comfortable. We also talk honestly about budget and geography.
We've visited the communities in our network. We know which ones have the right staffing for complex needs, which ones feel like home, and where the waitlists are. We make a recommendation โ not just a list.
We accompany you to tours and help you know what to look for. We help you hear past the marketing language to what's really being offered.
Once you've found the right place, we coordinate with the sending facility, the new community, the care team, and any home health or hospice providers. We take the logistics off your plate.
We check in after move-in to make sure your loved one is settling in well and the family feels supported. Our relationships don't end at the door.
One of the biggest fears families have about moving a loved one out of a skilled nursing facility is losing the care team they've built trust with. We understand that. And it's one of the reasons we've built our network carefully.
The communities we work with have access to care providers who operate across multiple settings โ including skilled nursing facilities and assisted living communities. In some cases, this means your loved one's care team doesn't have to change when they move.
Same faces. Same relationship. New home.
It's not a guarantee โ it depends on the specifics of each case โ but it's a real possibility we're proud to offer.
Independent living facilities (ILFs) are for seniors who are largely self-sufficient but want a community, amenities, and freedom from home maintenance. They range from large, resort-style campuses to smaller converted homes. No personal care assistance is included โ but it can often be brought in separately.
Assisted living facilities (ALFs) and Residential Care Facilities for the Elderly (RCFEs) are the same license category in California โ they provide hands-on help with daily activities like bathing, dressing, and medication. Board and care homes are small RCFEs (typically 6 or fewer residents) with a home-like feel and higher caregiver ratios. We'll help you figure out which is the right fit for where your loved one is today โ and where they may be heading.
It depends on your situation. In urgent cases โ when a discharge deadline is approaching โ we can move very quickly. In less urgent situations, we typically recommend taking a few weeks to explore options thoughtfully. Either way, we work at your pace.
That's exactly what we specialize in. We know which facilities in our network are equipped for higher levels of care, dementia, behavioral needs, and specialized medical requirements. We won't place someone somewhere that can't actually support them.
We understand this concern. We are paid by facilities when a placement is made โ but our long-term relationships depend entirely on making good matches. A bad placement hurts everyone, including us. We have no incentive to recommend a facility that isn't right for your loved one.
There's no commitment and no cost to start. Just reach out and tell us a little about your situation.
